第11届世界肺癌大会于2005年7月3日至6日于西班牙巴塞罗那举行,我中心王俊主任远赴西班牙参加了此次会议。我中心关于《应用RT-PCR法检测MAGE基因判断非小细胞肺癌患者淋巴结隐匿微转移》的海报被评为本届大会的杰出海报。
Detection of lymph node occult micrometastases in patients with non-small cell lung cancer by reverse transcriptase polymerase chain reaction on MAGE genes

Background: Occult micrometastases in lymph node in patients with non small cell lung cancer (NSCLC) is important in decision the treatment and prognosis of the patients. So far, there are no sensitive and specific methods can detect the occult micrometastases in lymph node. MAGE genes were recently confirmed only expressed in malignant cell and testis. We detected the lymph node occult micrometastases in patients with non-small cell lung cancer with reverse transcriptase polymerase chain reaction (RT-PCR) on MAGE genes.
Methods: 111 mediastinal lymph nodes from 53 patients with NSCLCand 40 mediastinal lymph nodes from 15 patients without cancer were studied to detect mRNA for MAGE-1,2,3 and -4 genes by RT-PCR assay and routine histological examination respectively.
Results:The occult micrometastases was recorded as positive when at least one of the four detected MAGE genes was confirmed expressed by RT-PCR. In the 40 lymph nodes from non-cancer patients,no expression of the MAGE genes were detected. In the 111 lymph node fromNSCLC patients, MAGE genes expressed in 46 lymph nodes,in another words,the occult micrometastases was 41.4% (46/111). With the routine histological examination, 27.9% (31/111) were found tumor invasion (p<0.05). For those 80 lymph nodes that show no tumor invasion by routine histological examination, the occult micrometastases detected by RT-PCR were 23.8% (19/80). But in the 31 lymph nodes with invasion of the tumor confirmed by routine histological examination, 27 (87.1%) were found the MAGE genes expression:which may due to that only about 85-90% NSCLC express the MAGE genes.
Conclusion:RT-PCR on MAGE genes is alternative method to detect the occult micrometastasis in lymph nodes of patients with NSCLC. The long-term meaning of this method need to be further evaluated.
